The Chronic Disease Management (CDM) Programme is a free HSE initiative that helps people manage their chronic diseases.
The Chronic Diseases Management Programme
The Chronic Diseases Management Programme is a free initiative by the HSE(Health Service Executive) and available to people over 18 years old who have a medical card or GP visit card and have a chronic disease or condition, like a stroke or TIA.
The aim of the program is to help you take control of your condition and tackle the risk factors that can make it worse with the support of your GP(family doctor)
Your GP (family doctor) will work with you to create a patient care plan that will make it easier to manage your disease in the long term.
Who can avail of the programme
The Chronic Diseases Management Programme open to anyone over 18 who has a medical card/GP visit card and one or more of the following conditions:
- stroke / TIA and irregular heartbeat (atrial fibrillation)
- cardiovascular disease, including heart failure, heart attack
- type 2 diabetes
- asthma
- chronic obstructive pulmonary (lung) disease (COPD)
To get a referral to join the Chronic Disease Management Programme, you first need to see your GP to confirm whether you meet the criteria. If you are eligible, they will organise the paperwork for you and may get the GP practice nurse involved.
